MDS Coordinator (LPN License Required)
Job SummaryWe are seeking an experienced MDS Coordinator to join our team at Liberty Healthcare & Rehabilitation Services.
Responsibilities- Develop and maintain a schedule of due dates for all MDS assessments.
- Coordinate the completion of MDS by all disciplines, ensuring accuracy and timeliness.
- Review MDS data for accuracy and collaborate with staff as needed to address any discrepancies.
- Complete MDS assessments and input into the computer system.
- Verify that assessments have been transmitted and approved by the State in a timely manner.
- Correct rejected records and prepare them for re-submission.
- Conduct CAAs according to state guidelines.
- Formulate Resident Care Plans (RCPs) in collaboration with the interdisciplinary care plan team.
- Schedule and conduct RCP meetings on a regular basis.
- Communicate RCPs to relevant staff members and notify supervisors of non-compliance.
- Collaborate with internal and external departments to ensure understanding and compliance with RCPs.
- Licensed Practical Nurse (LPN) with a current, valid license from the North Carolina Board of Nursing.
- Ability to read, understand, and follow facility policies, procedures, and local/state/federal requirements.
- Experience with MDS-RAP and Care Planning functions.
- Preferred experience with RUG-IV and MDS 3.0.
$45,000 - $60,000 per year, depending on experience and qualifications.
