Job Opportunity: Home Care Nurse

Interim HealthCare of the Twin Cities is seeking a skilled Home Care Nurse to join our team. As a Home Care Nurse, you will have the opportunity to work independently, providing high-quality care to patients in their own homes.

Key Responsibilities:

  • Participate in ongoing interdisciplinary assessment of the patient.
  • Develop and evaluate the plan of care in partnership with the patient, representative, and caregiver(s).
  • Provide services ordered by the physician as indicated in the plan of care.
  • Recognize and report changes in the patient's condition to the RN.
  • Counsel and educate the patient and family in meeting healthcare-related needs.
  • Complete timely and accurate clinical notes, including addressing patient progress.

Requirements:

  • Current license as a Licensed Practical Nurse (LPN) in the State of MN.
  • 2 years' experience as an LPN.
  • Able to travel within the geographic area serviced by the office.
  • CPR Certification.
