Home Health Physical Therapy Assistant

LHC Group is seeking a skilled Home Health Physical Therapy Assistant to join our team in Wethersfield, Connecticut.
About the Role:As a Home Health Physical Therapy Assistant, you will work under the supervision of a Physical Therapist to provide high-quality care to patients in their homes. Your primary responsibilities will include:
- Instructing and aiding patients in evidence-based treatment within the scope of physical therapy and according to state laws.
- Observing, recording, and reporting patient responses to treatment and changes in their condition to the supervising PT, nurse supervisor, and/or physician.
- Coordinating care with other healthcare team members as needed.
- Providing patient education and instruction in areas of therapy within your scope of practice.
To be successful in this role, you must have:
- A current license to assist in the practice of physical therapy in the state of Connecticut.
- Current CPR certification.
- A reliable vehicle and valid driver's license.
