Key Responsibilities:

  • Assess patient psychosocial needs, plan, and evaluate interventions to meet established patient goals
  • Collaborate with healthcare professionals to plan and evaluate team goals for each patient
  • Provide counseling and crisis intervention to patients with social or emotional problems
  • Facilitate patient referrals to community agencies when necessary
  • Provide financial counseling to achieve treatment goals
  • Respond to crisis situations, including domestic violence and child abuse
  • Provide witness services and evidence in court cases
  • Visit schools to assess academic concerns
  • Work with government agencies to help clients apply for Medicare and Medicaid
  • Arrange home visits to evaluate patient health, home environment, and family interaction

Requirements:

  • Master's degree in Social Work from a Council on Social Work Education accredited school
  • Active license as an LCSW in the state of practice
  • Two years of clinical experience in therapy, counseling, and case management
  • Brief and long-term psychotherapy experience
  • Cantonese, Mandarin, or Vietnamese language speaking
  • Compliance with professional standards of practice
  • Participation and completion of Amergis' Competency program
