Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse to join our team. As a LPN at Buckhead Center, you will play a vital role in providing high-quality care to our residents, working collaboratively with our interdisciplinary team to ensure their physical, emotional, and social needs are met.

Responsibilities

  • Provide direct care to residents, including administering medications, wound care, and vital sign monitoring
  • Conduct thorough assessments of residents' physical and emotional needs, developing and implementing individualized care plans
  • Collaborate with the interdisciplinary team to coordinate care and services, ensuring seamless transitions and continuity of care
  • Communicate effectively with residents, families, and healthcare professionals to ensure informed decision-making and optimal care outcomes
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices and policies

Qualifications

  • Current LPN licensure in the state of practice
  • Minimum 1 year of experience in long-term care or geriatric nursing
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively as part of a team
  • Strong problem-solving and critical thinking skills
