Job Summary

We are seeking a highly skilled Licensed Professional Counselor to join our Behavioral Health team at CommuniCare Health Center. As a key member of our team, you will provide high-quality behavioral health services to individuals, families, and groups, ensuring a comprehensive service plan is developed and implemented to meet individual patient needs.

Key Responsibilities
  • Provide screening, evaluation, diagnosis, and treatment to individuals, families, or groups, ensuring a comprehensive service plan is developed and implemented.
  • Document all encounters, including screening, individual, family, and group counseling sessions, and ensure accurate coding and billing of all patient encounters.
  • Function at or above the 90th percentile in productivity and provide excellent clinical outcomes based on patient panel.
  • Ensure adequate clinician coverage within the clinics during operating hours and provide quality control of the behavioral health department, including compliance with accreditation standards.
  • Provide exceptional clinical care based on patient satisfaction scores, clinical outcome measures, and chart audits, and ensure 100% participation in department chart audits.
  • Assist Behavioral Health clients through the use of the Center's established referral system for in-house services and to appropriate community agencies, while providing consultation to provider staff to ensure proper interdepartmental support.
  • Document all outcome measures for the BH program, including the Patient Health Questionnaire-9, the Vanderbilt ADHD Scale, and other outcome measures implemented in the course of practice.
  • Attend and actively participate in departmental meetings and Center activities to pursue the mission and vision of the Center.
  • Provide excellent internal/external customer service via telephone, fax, or face-to-face contact to assist patients with their health care needs and represent the Center in any outside communication or activities.
  • Establish and maintain a working relationship with community-based agencies in the best interest of the patient or the Center.
Requirements
  • A master's in counseling or a counseling-related field from an accredited college or university.
  • Licensed to practice in the State of Texas.
  • Thorough knowledge of parenting issues, communication skills, family dynamics, brief and focused psychotherapeutic interventions, such as cognitive behavioral and problem-based counseling.
  • Ability to work well with all staff, community, and volunteers.
  • Ability to plan and organize work with minimal supervision.
  • Knowledge of and sensitivity to cultural diversity.
  • Valid Driver's License in the State of Texas.
  • Preferred bilingual in English and Spanish.
  • Certified in Basic Life Support (BLS), American Heart Association.
  • 2 years of experience as a Counselor.
