Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at WellStone Inc. As a Case Manager, you will play a vital role in providing outpatient case management services to individuals with serious mental illness.

Key Responsibilities
  • Case Management Services: Provide direct services to clients to address unmet needs and provide information about the continuum of care.
  • Training and Education: Provide training on money management, basic living skills, and medication management to clients.
  • Collateral Services: Assist clients with transportation to appointments, referrals to agencies, and other support services.
  • Documentation and Record-Keeping: Maintain high-quality client documentation and records management using electronic charting.

Requirements
  • Education and Training: State of Alabama Adult Case Management Training.
  • Skills and Experience: Excellent oral and written communication skills, knowledge of community agencies and resources, and personal automobile liability insurance.
