Key Responsibilities- Case Management Services: Provide direct services to clients to address unmet needs and provide information about the continuum of care.
- Training and Education: Provide training on money management, basic living skills, and medication management to clients.
- Collateral Services: Assist clients with transportation to appointments, referrals to agencies, and other support services.
- Documentation and Record-Keeping: Maintain high-quality client documentation and records management using electronic charting.

- Education and Training: State of Alabama Adult Case Management Training.
- Skills and Experience: Excellent oral and written communication skills, knowledge of community agencies and resources, and personal automobile liability insurance.
