Job Description

We are seeking a highly skilled Pediatric Care Coordinator Team Member Opportunity to join our team at Carolina Health Centers, Inc. As a key member of our healthcare team, you will be responsible for providing comprehensive care coordination services to children and families in our community.

Key Responsibilities:

  • Care Coordination: Facilitate family access to medical home providers, staff, and resources; assist with or promote the identification of patients in the practice with special health care needs (such as CYSHCN) and add them to a registry used to plan and monitor care;
  • Assessment and Screening: Assess child/patient and family needs and unmet needs, strengths, and assets; screen for child's developmental progress and provide follow-up if a delay is identified;
  • Family Support: Initiate family contacts and create ongoing processes for families to determine/request the level of care coordination support they desire for their child/youth or family member at any given time;
  • Collaboration and Communication: Build care-relationships among family and team supporting the primary caregiving role of the family; develop care plans with family/youth/team (emergency plan, medical summary, and action plan as appropriate); execute care plans, evaluate effectiveness, monitor in a timely manner, and make changes as needed;

Requirements:

  • Education: Bachelor's Degree as a nurse, social worker, or the equivalent with appropriate past experience in health care and advanced training or education in child development, family studies, psychology, or related field;
  • Experience: Three (3) years relevant experience, or the equivalent, in community-based pediatrics or primary care, particularly in the care and service of vulnerable populations such as CYSHCN;
  • Skills: Excellent communication and interpersonal skills; ability to read, write, and communicate effectively orally and in writing; proficient in use of computer and keyboard; knowledge of HIPAA and ability to maintain confidentiality;

Work Environment:

This role requires occasional travel within our service area and moderate exposure to blood-borne pathogens. The noise level is that of an office and is typically quiet to moderate.

Reporting Relationships:

Responsible to: Directly supervised by Director of Early Childhood Services and has a dotted line of accountability to the Director of Pediatrics.
