Job Summary

The Nurse Practitioner (NP) supports our Adobe Provider Group (APG) in providing quality health care to the majority of counties in Southern and Northern Arizona. The APG NP has the overall responsibility of treating assigned members and meeting quality measurements outlined in the contract.

Key Responsibilities
  • Assess patient/family psychosocial needs, plan, implement, procure services for, and evaluate care needs while meeting with members in their homes.
  • Conduct in-home assessments and primary care on identified patients, following established guidelines.
  • Identify diagnoses to be used in care management and active medical management.
  • Formulate a list of current and past medical conditions using clinical knowledge, judgment, and current findings.
  • Identify quality measures outlined in the contract and ensure completion.
  • Identify gaps in care, internal and external referral needs, and urgent/emergent situations for proper intervention.
  • Educate members on topics related to health conditions.
  • Compliant with all HIPAA regulations and maintain the security of protected health information (PHI).
Requirements
  • Master's Degree in Nursing is required.
  • Current Licensed Nurse Practitioner for Arizona.
  • Current CPR certification.
  • Current TB Test.
  • Must maintain current CPR certification throughout employment.
Work Environment

The employee must occasionally lift and/or move more than 40 pounds. Must be able to physically perform the essential duties of the position which include lifting 40 lbs., transporting materials, stooping, kneeling, crouching, reaching, use of hands, balancing, walking, standing, talking, hearing, and typing.
