Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled Home Health Physical Therapist to join our team in Brighton, MA.

About the Role:

We are looking for a compassionate and experienced Physical Therapist to provide exceptional rehabilitative therapy to geriatric patients in Senior Living Communities.

Responsibilities:
  • Perform diagnostic tests and measurements to assess client mobility, strength, and balance.
  • Develop and implement individualized care plans to promote client independence and safety.
  • Collaborate with nursing staff to revise and update care plans as needed.
  • Provide education and instruction to clients and their families on rehabilitative care and activities.
  • Accurately document client care management and services provided.

Requirements:
