We are seeking a highly skilled and compassionate Neurology Nurse Navigator to join our team at Trinity Health. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and ensuring seamless transitions across all settings.
Key Responsibilities- Develop, implement, and evaluate clinical practice guidelines/care paths for patients.
- Collaborate with the healthcare team to provide quality patient care and facilitate the identification of potential care problems.
- Act as a liaison between patients and their families, physicians, staff, and other healthcare professionals to coordinate patient care and ensure high-quality outcomes.
- Proactively advocate for patients, working to overcome barriers and resolve concerns.
- Provide educational information and resources to patients to assist them in coping and fostering self-care activities.
- Review literature, methodology, outcomes, and implications of nursing/medical research and disseminate findings.
- Establish and maintain a knowledge base of ongoing clinical research activities.
- Graduate of an accredited school of nursing program required.
- Bachelor of Science in Nursing preferred.
- Current/valid RN license from the state licensing board required.
- Basic Life Support (BLS) or Advanced Care Life Support (ACLS) required.
