Job Summary

The Home Based Primary Care Program has base operations at the Jesse Brown VA Medical Center, the Auburn Gresham Community Based Outpatient Clinic (CBOC) in Chicago, Illinois, and the Adam Benjamin Jr CBOC in Crown Point, Indiana. This Nurse will be assigned to one of these locations and reports directly to the Nurse Manager for HBPC. This is a mobile position, requiring daily visits to Veterans in their homes, with other tasks being completed at one of the other locations, or from home.

Responsibilities
  • Provides direct and indirect nursing care and counseling to Veterans enrolled in the HBPC Program, and their families.
  • Provides for the promotion and maintenance of health and prevention of illness and injury evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counselling; provision of care supportive to or restorative of life and well-being; and executing medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider.
  • Provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the HBPC Program.
  • Responsible for case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning.
  • Plans and delivers technically competent care in the home of Veterans with complex needs, including health promotion, health prevention, geriatric care, primary care, and palliative care.
  • Utilizes interventions, which maximize health capabilities and reduces unnecessary urgent care and emergency room visits, per Veteran's goals of care.
  • Evaluates outpatient outcomes of evidence-based decisions and practice changes for HBPC individuals, groups, and populations.
  • Responsible for the documented HBPC outcomes at the program or service level.
Requirements
  • U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
  • Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure.
