Physical Therapist for Stratford Manor in West Orange

Job Description

As a Physical Therapist at Stratford Manor, you will be an integral member of our team, working collaboratively with the therapy teams and Rehab Department to provide high-quality care to our residents.

Responsibilities
  • Evaluate and document resident function, gait, strength, ROM, balance, and mobility
  • Develop and implement individualized treatment plans to improve resident outcomes
  • Provide therapy as outlined in treatment plans and prescriptions
  • Offer treatments that help improve patient inabilities and/or challenges
  • Manage workflow, including documentation of individual and group treatments and notes in the electronic records system
  • Familiar with PDPM and align therapy treatments and plans with understanding of Medicaid and Medicare billing
Qualifications
  • New Jersey License
  • Background/experience in long-term care (specifically a Skilled Nursing Facility)
  • Strong organizational, interpersonal, and communication skills
  • Ability to manage patients with different types of personalities and ability levels
  • Creative problem-solving skills
