We're seeking experienced LPNs or RNs who are passionate about delivering high-quality patient care in a home health setting. Our team serves pediatric, adult, and geriatric patients throughout the state of DE, offering a wide range of medical services including medication administration, gastric tube feedings, tracheostomy maintenance, ventilator support, and more.

Key Responsibilities:

  • Deliver personalized care to medically fragile patients
  • Coordinate with families, physicians, and other healthcare providers to ensure seamless care
  • Build strong relationships through exceptional customer service and communication
  • Work a minimum of two weekend shifts per month
  • Administer medications and prescribed treatments as ordered

Requirements and Qualifications:

  • DE LPN or RN license with one year of experience in a healthcare setting (preferred)
  • Able to attend a two-week orientation in our Camden office, followed by patient field orientation
  • Previous home health and/or Long Term Care experience a plus
  • Tracheostomy and ventilator experience preferred
