Job Title: Pharmacy Services Coordinator

Estimated Salary: $50,000 - $70,000 per year

We are seeking a highly organized and detail-focused Pharmacy Services Coordinator to join our team at PACE Southeast Michigan. As a key member of our clinical pharmacy services team, you will be responsible for assisting the Clinical Pharmacist in providing comprehensive, customer-oriented services to our participants.

Responsibilities:

  • Assist the Clinical Pharmacist in identifying and resolving drug-related problems for participants.
  • Work closely with the Enrollment team to ensure accurate medication reconciliation and onboarding of new participants.
  • Support Fraud, Waste and Abuse efforts by preparing detailed enrollment grids and communicating with dispensing pharmacies.
  • Reconcile allergies and immunizations and update pharmacy demographics for all participants.
  • Prepare initial and annual Comprehensive Medication Review packets using clinical criteria.
  • Review, execute, and communicate on refill requests from dispensing pharmacies.
  • Communicate with skilled nursing facilities for medication administration records and reports as assigned.
  • Attend weekly huddles and meetings as assigned.

Requirements:

  • High school diploma or GED required.
  • Current Pharmacy Technician (CPhT) license with the State of Michigan preferred.
  • Previous experience of at least 6 months preferred.
  • Proficient computer and typing skills.
  • Ability to operate a calculator and use intermediate math skills.
  • Ability to communicate clearly both in-person and over the phone.
  • Ability to work effectively with others.
  • Ability to multitask and manage multiple projects simultaneously.
