Job Details

Requirements
  • Home health experience preferred but not required
  • NY Physical Therapist License or eligible
  • Evaluate patient needs and develop treatment plans
  • Provide care and treatment based on physician care plan and agency policy
  • Instruct family members and caregivers in physical therapy techniques

Certifications required: CPR/First Aid
