About the Role

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Home Health team. As a Home Health LPN, you will provide high-quality patient care in the comfort of their own homes.

Key Responsibilities:

  • Provide professional nursing services under the direction of a RN in compliance with state and federal regulations.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Confer with RN Supervisor regarding needed changes in the Plan of Treatment.
  • Communicate effectively with patients, physicians, and other healthcare team members.

Requirements:

  • Graduate of an accredited LPN program.
  • Current nursing license in the practicing state.
  • Valid drivers license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.
