As a Private Duty Nurse, you will provide one-on-one care to our patients in the comfort of their own homes, assisting them with transitioning from hospital to home and providing the continuous care needed to remain at home with their loved ones.
Responsibilities- Implement physicians' prescribed plans of care
- Conduct patient assessments and coordinate care
- Administer prescribed medication, treatments, and therapies
- Document care provided in a timely and compliant manner
- Provide health education and training to patients and their families
- Collaborate with the healthcare team to ensure seamless care delivery
- Active, unencumbered LPN/LVN license in the state of practice
- Current CPR certification
- TB skin test (current within the last 12 months)
- At least six months of recent experience as a Licensed Nurse in a clinical care setting
