Key Responsibilities- Conduct Comprehensive Health Assessments: Evaluate patients' vital signs, medication adherence, and overall health status to identify areas for improvement.
- Develop Individualized Care Plans: Collaborate with the healthcare team to create tailored care plans that meet patients' unique needs and goals.
- Administer Medications and Treatments: Safely administer medications, treatments, wound care, and medical procedures in accordance with prescribed protocols and care plans.
- Monitor and Document Patient Progress: Regularly assess and document patient condition changes, communicating updates to the healthcare team as needed.
- Education and Support: Educate patients and their families on disease management, medication adherence, and lifestyle adjustments to promote wellness and independence.
- Interdisciplinary Collaboration: Work closely with physical therapists, occupational therapists, social workers, and other healthcare professionals to ensure a holistic approach to patient care.
- Current LVN License: Possess a current LVN license in the state of Texas.
- Home Health Experience: Proven experience as an LVN in a home health setting, with a strong understanding of clinical assessment and patient care.
- Strong Communication Skills: Possess excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Independence and Teamwork: Demonstrate the ability to work independently and as part of a team, with a strong commitment to delivering high-quality patient care.
- Valid Driver's License: Possess a valid driver's license and reliable transportation.
