Job Title: Physical Therapist - Home Health

Advanced Home Health Care of Cincinnati is seeking a skilled Physical Therapist to join our Home Health team. As a Physical Therapist, you will be responsible for providing physical therapy services to clients in their homes, in accordance with the plan of care and Agency policies and procedures.

Responsibilities:
  • Perform physical therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluation of clients who are receiving services under a medically approved plan of care.
  • Develop a therapy care plan to provide direction to other health team members involved in the plan of care.
  • Document all findings, plans, interventions, and client progress (outcomes) on designed forms and submit per Agency guidelines within forty-eight (48) hours.
  • Communicate plans and changes to the physician and to case manager and other Agency staff through the care plan, written progress notes, and participation in care conferences.
Requirements:
  • Graduate of physical therapy program.
  • Possesses current registration or license as required by the state of practice.
  • Minimum of two (2) year experience in home health or a health care facility preferred.
  • Demonstrates good written and verbal communication skills, interpersonal skills.
  • Licensed driver with automobile insured in accordance with state/Agency requirements.
  • Good driving record and reliable transportation.
  • Current CPR certification.
