Job Title: RN - Long Term Care

We are seeking a skilled Registered Nurse to provide exceptional care to our residents in a long-term care setting. As a member of our healthcare team, you will be responsible for delivering high-quality patient care, collaborating with colleagues, and promoting a positive and supportive environment.

Job Summary:

This is a part-time Registered Nurse position that requires a strong commitment to providing person-centered care. You will work closely with our interdisciplinary team to develop and implement individualized care plans, ensuring the physical, emotional, and social needs of our residents are met.

Responsibilities:
  • Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs
  • Conduct thorough assessments, develop care plans, and implement interventions to promote resident well-being
  • Collaborate with colleagues to ensure seamless care transitions and effective communication
  • Participate in quality improvement initiatives and contribute to a culture of excellence
  • Document resident care, progress, and concerns in a timely and accurate manner
Requirements:
  • Current RN licensure in the state of Illinois
  • Experience in long-term care or a related field
  • Strong communication and interpersonal skills
  • Ability to work effectively in a team environment
