Job Summary

We are seeking a highly skilled Patient Logistics Coordinator to join our team at CommonSpirit Health. As a key member of our Command Center, you will play a critical role in facilitating the placement of patients within our Dignity Health facilities and specialized care settings.

Key Responsibilities
  • Work independently to coordinate the transfer of patients to the appropriate facility and unit
  • Communicate effectively with referring and receiving facility staff to ensure timely placement of patients
  • Identify potential problems and bring them to the attention of the Supervisor, ANS, and/or Leadership to prevent and/or resolve barriers to timely placement and/or transfer of patients
  • Document activities clearly and accurately to promote effective communication with members of the care team
Requirements
  • Experience in a healthcare setting, preferably in bed control and/or transfer center capacity
  • Ability to coordinate effectively with a variety of customers, including providers, hospital and office staff, health plans, medical groups, internal departments, community resources, and peers
  • Knowledge of levels of care and basic medical terminology
  • Ability to work independently and apply critical thinking skills toward making appropriate decisions, as well as in a team environment
