Position Summary:

The Client Services Coordinator plays a crucial role in advocating for clients, facilitating the integration of services, and ensuring continuity of care. This position requires a strong ability to connect clients with necessary resources and coordinate multiple services to achieve optimal outcomes. Key responsibilities include conducting assessments, developing service plans, coordinating and linking services, performing follow-ups, and advocating for clients' needs.

Key Responsibilities:
  • Conduct thorough assessments to identify client needs and develop tailored service plans.
  • Coordinate and link clients to essential medical, social, educational, and therapeutic services.
  • Monitor and follow up on service delivery to ensure effectiveness and client satisfaction.
  • Advocate for clients to access necessary resources and support systems.
Qualifications:
  • Bachelor's Degree in a relevant human services field.
  • A minimum of one year of experience in a related field.
  • Certification as a Qualified Mental Health Professional (QMHP), Qualified Intellectual Disabilities Professional (QIDP), or Certified Mental Health Professional (CMHP) is preferred.
  • Availability for on-call emergency services as needed.
