Job Description
This is a per diem opportunity with a competitive hourly rate. As a RN in Home and Community Based Services, you will provide personalized care to patients in their homes, ensuring their comfort, safety, and well-being. You will work closely with patients and their families, collaborating with other healthcare professionals to deliver comprehensive care.
Responsibilities
* Develop and implement individualized plans of care, taking into account patient needs and preferences
* Conduct regular assessments to monitor patient progress and adjust care plans accordingly
* Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
* Adhere to infection control standards and maintain a clean, safe environment for patients
Requirements
* A minimum of one year experience as an RN preferred
* Current RN licensure in state of practice
* Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
* Current CPR certification required
