Responsibilities
  • Resident Care
    • Provide individualized care to residents, addressing their unique needs and preferences.
    • Monitor residents' health status and adjust care plans as needed.
  • Communication and Collaboration
    • Work effectively with other healthcare professionals to ensure seamless care transitions.
    • Communicate resident information and concerns to family members and caregivers.
Requirements
  • Licensure and Education
    • Current Pennsylvania state license to practice as a Licensed Practical Nurse (LPN).
    • Completion of an approved LPN educational program.
  • Experience and Abilities
    • Minimum of 6 months of experience as an LPN or in a related role.
    • Ability to lift a minimum of 50 lbs.
