Job Summary

Innovative Renal Care is seeking a highly skilled Master Social Worker to join our team. As a Patient Care Specialist, you will play a critical role in providing exceptional care to patients with End Stage Renal Disease.

Key Responsibilities

  • Assess and educate patients on their rights and responsibilities, as well as guide them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.
  • Collaborate with patients on care planning, ensuring their needs are met and their goals are achieved.
  • Develop and implement individualized care plans that address the unique needs of each patient.
  • Provide emotional support and counseling to patients and their families, helping them cope with the challenges of End Stage Renal Disease.
  • Work closely with the healthcare team to ensure seamless care coordination and communication.

Requirements

  • Master's degree from an accredited school of social work (MSW).
  • Current state licensure (e.g. LCSW, LMSW), requirements for the state of practice.
  • Two years of clinical work experience, preferably in an ESRD setting.
  • Excellent communication and interpersonal skills.
