Job Summary
Innovative Renal Care is seeking a highly skilled Master Social Worker to join our team. As a Patient Care Specialist, you will play a critical role in providing exceptional care to patients with End Stage Renal Disease.
Key Responsibilities
- Assess and educate patients on their rights and responsibilities, as well as guide them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.
- Collaborate with patients on care planning, ensuring their needs are met and their goals are achieved.
- Develop and implement individualized care plans that address the unique needs of each patient.
- Provide emotional support and counseling to patients and their families, helping them cope with the challenges of End Stage Renal Disease.
- Work closely with the healthcare team to ensure seamless care coordination and communication.
Requirements
- Master's degree from an accredited school of social work (MSW).
- Current state licensure (e.g. LCSW, LMSW), requirements for the state of practice.
- Two years of clinical work experience, preferably in an ESRD setting.
- Excellent communication and interpersonal skills.
