Senior Care Nurse Opportunity

Your Tailor Made Senior Service is seeking a skilled Registered Nurse to provide quality, comprehensive nursing care in the patient's home as prescribed by the physician in the plan of care.

Responsibilities:
  • Provide quality, comprehensive nursing care in the patient's home as prescribed by the physician in the plan of care
  • Ensure quality and safe delivery of home health care services in compliance with all applicable laws, rules, regulations, and policies
  • Perform nursing assessments and develop, implement, and evaluate plans of care in conjunction with the physician, patient, and caregivers per agency policy and procedure
  • Evaluate the effectiveness of the plan of care, the patient's response to care, and outcomes of care and revise the plan of care as needed
  • Initiate appropriate preventive and rehabilitative nursing procedures following the physician's plan of care
  • Document all communications with the patient, family, physician, CTM, pharmacy, other disciplines, and appropriate others as indicated on communication notes
  • Prepare a written plan for the Home Health Aide to follow, if applicable
  • Collaborate with the interdisciplinary team to develop the plan of care for the patient
  • Manage nursing treatments and activities to meet the needs of each individual patient
  • Evaluate utilization of patient services and patient progress
  • Review evaluation with the care team, patient, family members, physician, and case manager as needed
Requirements:
  • Valid RN license in Texas
  • OASIS/Medicare Certified Visits Experience
  • Electronic Charting experience - We use Finale Health
  • Developed computer skills
  • Minimum of 1 year of recent home health nursing experience
  • Driver's License and Vehicle with Current Auto Insurance
  • American Heart Association BLS
  • Valid U.S. Driver's License
  • Problem solving skills
  • Excellent Communication Skills
  • Positive, Professional attitude
