About the Job

BAYADA Home Health Care is seeking a skilled and compassionate Physical Therapist to join our team in providing high-quality home health care services to clients in Thornton, Pennsylvania.

Job Summary

This full-time opportunity involves performing outpatient home health visits for our senior living office, serving adult and geriatric clients in Delaware County. As a licensed Physical Therapist, you will be an integral part of a multidisciplinary healthcare team, providing rehabilitative care and skilled nursing to clients, enabling them to receive the medical attention required to remain at home.

Responsibilities
  • Conduct outpatient visits to clients in designated geographic territories
  • Perform diagnostic tests and measurements, such as mobility assessments, transfer status evaluations, and safety assessments
  • Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance interventions, and other rehabilitative activities
  • Continuously assess and revise care plans, collaborating with nursing staff as necessary
  • Educate and instruct clients, family members, or caregivers on rehabilitative care and activities promoting client independence
  • Accurately document observations, interventions, and evaluations related to client care management and services provided
Requirements
  • CURRENT Pennsylvania Physical Therapist license
  • MINIMUM one year of recent work experience as a Physical Therapist
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
  • Ability to work independently and manage time effectively
  • Strong interpersonal, organizational, and problem-solving skills
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred
