MDS Nurse Coordinator - Full-time: On-site, Not remote.

This role is pivotal in overseeing the formulation and execution of the Resident Assessment Instrument through the completion of the Minimum Data Set (MDS) in alignment with state and federal regulations as well as the operational policies of the facility. The individual in this position plays a crucial role within the interdisciplinary team, contributing to the assurance of care quality, service continuity, and tailored patient-centered objectives.
  • Performs comprehensive patient evaluations to ascertain health status, required care levels, and any necessary adjustments
  • Guarantees precise and prompt completion of the RAI
  • Facilitates interdisciplinary involvement in the RAI and resident care planning
  • Assists in the creation and execution of initiatives and protocols aimed at optimizing the facility's reimbursement capabilities while enhancing care quality

Qualifications

  • Current active RN licensure in NJ
  • Experience as an MDS Coordinator
  • Proficient in MDS Completion
  • Familiarity with Medicare and Medicaid regulations and benefit criteria
