We're seeking a highly skilled Advanced Practice Provider to join our team at ChenMed. As a key member of our healthcare team, you will be responsible for providing exceptional patient care through virtual consultations and remote diagnostic services.
Key Responsibilities- Assess and diagnose patients' physical and psychosocial health status through virtual consultations
- Develop and implement patient management plans based on interpretation of findings
- Evaluate need for immediate nursing intervention, consultation, and/or referral
- Collaborate with patients' primary care physicians and other medical providers to ensure comprehensive care
- Communicate effectively with patients and families to promote positive health habits and outcomes
- Bachelor's degree in Nursing (BSN) and graduate of a school of nursing for Advanced Practice Nursing with certification in area of specialty
- Minimum 3 years' acute/primary care clinical work experience
- Minimum 2 years' telehealth work experience preferred
- Spoken and written fluency in English; bilingual (Spanish/Creole) a plus
