Job Title: Nursing Case Manager

At InfuCare Rx Inc on behalf of itself and its subsidiaries, we are seeking a highly skilled and experienced Nursing Case Manager to join our team. This is a unique opportunity to work in a dynamic and growing organization that provides specialized home infusion services to patients in need of IV therapy and specialty pharmacy services.

Job Summary:

The Nursing Case Manager will serve as the lead nursing staff member of the interdisciplinary clinical patient care team, responsible for referral management, RN home visit scheduling, and ongoing patient care management in collaboration with infusion office pharmacists, Intake team members, and Dietitians.

Key Responsibilities:
  • Support hospital case managers, prescribers, home health agencies, LTC facilities, liaisons, and other referral sources to coordinate patient discharges to home infusion services.
  • Oversee home infusion care provided to patients with acute and chronic diseases.
  • Develop and maintain knowledge of IV access devices, patient assessments, multidisciplinary care planning, patient education, IV medication administration, laboratory values, and ongoing clinical patient monitoring.
  • Foster new and existing relationships with industry healthcare professionals for educational purposes and potential partnerships.
  • Identify patient and caregiver home infusion care appropriateness, patient response to care, and help develop nursing care plans in partnership with prescribers, RN field staff, and other members of the patient's healthcare team to ensure optimal patient outcomes.
  • Remain current on new therapies, industry trends, and accreditation standards for home infusion services.
Requirements:
  • Current registered nurse license in good standing.
  • Bachelor's degree and CRNI preferred.
  • A minimum of 3 years home infusion nursing experience is required.
  • 1-2 years supervisory experience in a home care setting preferred.
  • Working knowledge of insurance industry requirements preferred.
  • Practical knowledge of State and Federal regulatory requirements and knowledge of ACHC regulations as it applies to infusion home care is essential.
  • Must have strong organizational skills and liaison capabilities with well-developed written and verbal communication skills.
  • Able to work independently with minimal supervision.
  • Minimal travel in service area may be required.
Physical Demands:

Required to stand, walk, sit, talk, and hear; required to use hands to operate vehicles and office equipment; must be able to occasionally lift and/or move up to 25 lbs; specific vision requirements for this job include: close, distance, and peripheral vision, and the ability to adjust focus.
