Home Health Physical Therapist

We are seeking a skilled and compassionate Physical Therapist to join our team in providing high-quality home health care services to our patients in the Mt. Vernon and surrounding areas.
About the Role:- Assess and evaluate patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.
- Develop and implement individualized treatment plans to improve and enhance patient well-being.
- Provide services within the scope of practice as defined by state laws governing the practice of physical therapy, in accordance with the plan of care, and in coordination with other members of the healthcare team.
- Collaborate with the physician in the development of the therapy plan of care and observe and report patient response to treatment and any changes in condition to the nurse supervisor and/or physician.
- Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice and evidence-based techniques.
- Instruct and inform participating members of the healthcare team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
- Visit patients within the entire service area of their respective agency and other nearby LHC agencies as needed.
- Complete progress notes for each visit and submit them per LHC policy.
- Current Physical Therapy licensure in the state of practice.
- Current CPR certification.
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
