Job Summary:

Liberty Hospice seeks an experienced Hospice Care Specialist to provide direct patient care services, conduct clinical assessments, and develop individualized care plans. The ideal candidate will have a strong understanding of psycho-social principles, family dynamics, and community resource planning.

Responsibilities:

  • Provide direct patient care services and document patient care data
  • Conduct clinical assessments to develop individualized care plans and set treatment goals
  • Apply knowledge of psycho-social principles and family dynamics to identify and assess factors affecting medical treatment and desired outcomes
  • Communicate with agency personnel and coordinate with other services to provide optimum care
  • Assist with homecare visits as needed
  • Relate well to patients and families at all levels, responding professionally to verbal and nonverbal communicators
  • Evaluate bereavement needs of families through initial assessments and interdisciplinary team coordination
  • Provide bereavement support and grief care to patients and caregivers

Requirements:

  • Masters of Social Work degree from a school accredited by the Council on Social Work Education, with a minimum of one year practical experience in home health/hospice care
  • Strong interpersonal skills, with the ability to effectively communicate within a multidisciplinary setting
  • Accept travel and working conditions as assigned
  • Minimum of twelve hours of continuing education annually

Benefits:

We offer a competitive salary range of $65,000 - $85,000 per annum, depending on experience.
