Job Summary

The Physical Therapist is responsible for providing physical therapy services to patients in their place of residence, in accordance with physician orders and the individualized plan of care under the direction and supervision of the Director/Manager of patient services.

Key Responsibilities
  • Develop and implement individualized plans of care in partnership with patients, representatives, and caregivers.
  • Provide patient, caregiver, and family counseling and education.
  • Prepare clinical notes and progress summaries based on goal attainment.
  • Communicate with physicians and other healthcare practitioners regarding patient care.
  • Provide physical therapy services to patients according to written physician orders and IDG-prescribed physical therapy.
Additional Responsibilities
  • Assess and evaluate therapeutic, rehabilitative, and functional status, and participate in the development of the total plan of care.
  • Direct physical therapy treatment and instruct patients and families/caregivers in the use and care of therapeutic appliances.
  • Identify patient and family/caregiver needs for other home health services and refer as necessary.
  • Consult with physicians and IDG regarding changes in treatment.
  • Participate in discharge planning and provide in-service education programs for nursing organization personnel.
  • Perform home safety assessments and participate in peer consultation and quality assessment processes.
Requirements
  • Licensed to practice as a physical therapist within the state, if applicable.
  • Passed an examination for physical therapists approved by the state and possesses a degree from a baccalaureate or master's program in physical therapy approved by CAPTE or any successor organization.
  • Two years of appropriate experience as a physical therapist, with community/home health experience preferred.
  • Ability to exercise initiative and independent judgment, and demonstrate good verbal and written communication and organization skills.
  • Possesses and maintains current CPR certification and is a licensed driver with an automobile that is insured in accordance with state or organization requirements.
