Compensation and Benefits
The compensation for this role ranges from $145,600.00 to $160,160.00 annually. Additionally, housing accommodations are provided at a competitive rate of $200 per week, with a one-time non-refundable cleaning fee of $200. Benefits include:

Key Responsibilities
The Clinical Case Manager will be responsible for:
- Planning, organizing, and coordinating swing bed admissions.
- Reviewing emergency department admissions and discharges.
- Providing administrative support and input for process improvement.
- Collaborating with referral specialists and managing medical records.
- Conducting financial assessments to ensure timely payment for services rendered.
- Responding to inquiries from healthcare providers, families, and external facilities.
- Delivering case management, social services, utilization review, and discharge planning.
- Identifying and coordinating swing admissions while verifying admission requirements.
- Engaging with specialty offices to facilitate transfers and care collaboration.
- Participating in interdisciplinary team meetings and utilization review discussions.
- Assessing patient needs for intervention and community resources.
Qualifications
Applicants should possess:
- Three to five years of clinical nursing experience.
- One to two years of experience in case management or a related field is preferred.
- Current nursing licensure in California.
- A Bachelor's degree in nursing is highly preferred.
- BLS certification is required; ACLS certification is preferred.
- A High School Diploma or GED.
- Mandatory attendance for the initial training period.
