We're seeking a dedicated and compassionate Physical Therapist to join our Home Health team in Limestone/Freestone/Navarro/McLennan. This is an exciting opportunity to be part of a dynamic organization that prioritizes patient-centered care and teamwork.

**What You Can Expect**

The successful candidate will enjoy a salary range of $90,000 - $110,000 per year, plus a one-time sign-on bonus of $20,000. Our comprehensive benefits package includes medical, dental, and vision coverage, paid time off, professional development opportunities, and a company-matching 401(k) plan.

**Key Responsibilities**

As a Home Health Physical Therapist, you will:

  • Work closely with patients in their homes, developing and implementing individualized treatment plans to address their unique needs and goals
  • Collaborate with other healthcare professionals to ensure seamless care coordination and optimal patient outcomes
  • Conduct thorough assessments and evaluations to identify areas for improvement and measure progress
  • Communicate effectively with patients, families, and caregivers to ensure mutual understanding and respect

**Qualifications**

To succeed in this role, you must meet the following requirements:

  • A Master's degree in physical therapy from a CAPTE-accredited program
  • Current licensure to practice as a physical therapist within the state of agency operation
  • Valid CPR certification
  • A valid driver's license for travel to business locations
