We are seeking a highly skilled and compassionate LPN to join our team as a Medication Care Manager at Sunrise Senior Living. As a key member of our care team, you will be responsible for providing high-quality care and services to our residents by administering medication and treatments in a safe and organized manner.
Responsibilities- Administer medications and treatments to residents in accordance with their individualized care plans and physician orders.
- Document and maintain accurate records of medication administration, including resident medication profiles and medication pass records.
- Conduct regular medication reviews and updates with residents and their families to ensure that their medication needs are being met.
- Collaborate with the Resident Care Director and other members of the care team to develop and implement individualized care plans that meet the unique needs of each resident.
- Participate in ongoing education and training to stay current on the latest medication management practices and regulations.
- Contribute to a positive and supportive work environment by demonstrating a commitment to teamwork, communication, and resident-centered care.
- Current LPN licensure in the state of employment.
- Minimum of 1 year of experience in a long-term care or senior living setting.
- Strong knowledge of medication management principles and practices.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.
