Aya Healthcare is seeking a skilled Travel Licensed Clinical Social Worker to join our team in Santa Monica, CA. As a Clinical Social Worker, you will be responsible for providing high-quality patient care and support to patients and their families. Your expertise will be essential in assessing patient needs, developing care plans, and coordinating services to ensure seamless transitions.

Responsibilities:

  • Assess patient needs: Conduct thorough biopsychosocial assessments to identify patient needs and develop individualized care plans.
  • Develop care plans: Collaborate with healthcare teams to develop and implement care plans that meet patient needs and promote optimal outcomes.
  • Coordinate services: Coordinate services to ensure smooth transitions and facilitate patient care, including home care and equipment following discharge.
  • Communicate with patients and families: Provide education and support to patients and their families to facilitate discharge and contingency planning.
  • Participate in care conferences: Participate in care conferences to ensure that patient needs are met and care plans are updated as necessary.
  • Document patient information: Document patient information to facilitate care coordination and ensure accurate records.
