Family Medicine Physician

At Proactive MD, we believe that a healthy workforce is a productive workforce. That's why we're seeking a talented Family Medicine Physician to join our team at the Health and Wellness Center. As a key member of our team, you will be responsible for providing patient-centered comprehensive primary care services, including performing episodic, preventive, and/or primary health care, developing personalized treatment plans, and conducting physical exams and health-risk assessments.
Key Responsibilities:
- Oversee and provide patient-centered comprehensive primary care services
- Develop personalized treatment plans in collaboration with patients
- Conduct physical exams, health-risk assessments, and history reviews
- Provide referrals to appropriate medical resources or specialty providers as needed
- Participate in the development of annual Health and Wellness Center goals and objectives
- Work in tandem with a medical assistant, patient advocate, and services director to ensure a collaborative, team-oriented environment
- Support, lead, and model the vision, mission, and values of the company
Requirements:
- 1+ years post-residency experience
- Current and valid state medical license
- Board certification or eligibility in Family Medicine
- Excellent interpersonal skills
- Ability to communicate effectively and maintain working relationships with people from diverse backgrounds
- Knowledge of the healthcare industry and available patient resources
- Knowledge of HIPAA confidentiality requirements
- Current BLS or ACLS certification
