Nurse Practitioner

We are seeking a highly skilled and compassionate Registered Nurse to join our team at Community Rehabilitation Hospital North. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional patient care and support to individuals in need of continued rehabilitation services.

What You'll Do:
- Conduct thorough patient assessments and develop individualized care plans to address their unique needs and goals.
- Collaborate with our interdisciplinary team to provide comprehensive care and support to patients and their families.
- Administer medications and treatments as prescribed by physicians, and monitor patients' responses to ensure optimal outcomes.
- Document patient progress and communicate effectively with healthcare team members to ensure seamless care coordination.
- Participate in ongoing education and training to stay current with best practices and advancements in rehabilitation medicine.
Qualifications:
- Graduation from an accredited Bachelor of Science in Nursing or Associate Degree in Nursing program.
- Current state licensure as a Registered Nurse.
- CPR/BCLS certification, with ACLS preferred.
- Minimum six months' experience in an acute care setting, with a focus on rehabilitation medicine.
- Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
