We are seeking a skilled and compassionate Licensed Practical Nurse to join our team at CenterWell Home Health. As a key member of our care team, you will provide high-quality nursing care to patients in their homes, promoting independence, dignity, and well-being.
Key Responsibilities:
- Deliver professional nursing services under the direction of a RN, adhering to regulatory standards and company policies.
- Provide skilled nursing interventions in the treatment of patients' illnesses, rehabilitative needs, and preventative care.
- Maintain open communication with patients, families, and healthcare team members to ensure seamless care coordination.
- Confer with RN Supervisor regarding needed changes in the Plan of Treatment and accept verbal orders from physicians as permitted by state law/regulations/Nurse Practice Act.
Requirements:
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid drivers license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
