Key Responsibilities:- Provide direct patient care in a Skilled Nursing setting, adhering to established standards and protocols.
- Conduct thorough assessments, develop, and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with the healthcare team to ensure seamless care transitions and effective communication.
- Maintain accurate and detailed records of patient progress and care provided.
- Current Pennsylvania license as an LPN or LVN.
- AHA BLS certification.
- Additional certifications related to Skilled Nursing, such as CNA or CMA.
- At least 2 years of recent experience in Skilled Nursing.
