As a key member of our patient care team, you will provide high-quality care to patients in their own homes, working collaboratively with our clinical staff to ensure seamless delivery of care.
">About the Role
">We are seeking a skilled and caring Homecare Aide to assist with daily living activities, such as bathing, dressing, and grooming, as well as providing vital signs and administering medications as needed. You will work closely with our multidisciplinary team to develop and implement personalized care plans tailored to each patient's unique needs and goals.
">Key Responsibilities

- Provide direct patient care, including assistance with ADLs (Activities of Daily Living), vital sign monitoring, and administration of medications">
- Assist with wound care, range of motion exercises, and other therapies as prescribed by healthcare professionals">
- Document all interactions and care provided in electronic medical records">
- Participate in multidisciplinary team meetings to discuss patient progress and adjust care plans as needed">
Requirements
">To be considered for this role, you must have:

- A high school diploma or equivalent">
- CPR certification and a current, unrestricted certification as a certified nursing assistant (CNA) or home health aide (HHA)">
- Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals">
- A valid driver's license and a clean driving record">
- Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures">
