About the Role

We are seeking a highly skilled and compassionate Patient Care Manager to join our team at Heartland Hospice. As a key member of our clinical leadership team, you will be responsible for the day-to-day operations of our hospice sites, ensuring the provision of high-quality care to our patients and their families.

Key Responsibilities
  • Manage and oversee clinical and administrative functions at our hospice sites
  • Administer the clinical aspects of our hospice program, ensuring compliance with regulatory guidelines and managed care principles
  • Provide direct skilled bedside nursing care in patients' homes, as needed
  • Respond to phone referrals and inquiries in a timely and professional manner
  • Participate in on-call nursing schedules and manage site operations in the absence of senior leadership
Requirements
  • Registered Nurse with a minimum of Associate's Degree in Nursing required
  • Three years' experience in direct patient care in a clinical setting required
  • Minimum one year of management/supervisory experience required
  • Two or more years' experience in hospice or home care required
  • Current license to practice nursing in the state of employment required
