LHC Group is seeking a skilled Physical Therapist to join our team in providing high-quality, patient-centered care in the comfort of patients' homes.

Job Summary

We are looking for a compassionate and dedicated Physical Therapist to work with our patients in Kingston, Lake Katrine, and surrounding areas. As a Physical Therapist with LHC Group, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities

The Physical Therapist will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment and evaluation, the Physical Therapist will work with the patient and their family to develop a personalized treatment plan, perform interventions aimed at improving and enhancing the patient's well-being, and evaluate the patient's progress.

  • Evaluate patient's functional status and physical therapy needs, assist physician in developing therapy plan of care, and observe and report patient's response to treatment and any changes in condition to supervising nurse and/or physician.
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice, and instruct and inform participating members of healthcare team, patient, and family/caregivers regarding plan of care, functional limitations, and progress toward goals.
Requirements

To be considered for this role, you must have a current Physical Therapy licensure in the state of New York, current CPR certification, and a valid driver's license and vehicle insurance. Additionally, you must have access to a dependable vehicle.
