Senior LIFE Social Worker Job Description

Senior LIFE is a pioneering home and community-based program that provides comprehensive healthcare services and support to seniors living in their communities. Our mission is to empower seniors to remain at home, surrounded by the comfort and familiarity of their surroundings, while receiving the care they need.

Key Responsibilities:
  • Participate in interdisciplinary teams to assess and develop Plans of Care for LIFE Center members.
  • Oversee social work case management services for caseloads of up to 40 members, providing screenings, counseling, and external referrals as needed.
  • Reevaluate and update Plans of Care based on changes in members' status, emergent conditions, and other relevant circumstances.
  • Conduct ongoing assessments to ensure a thorough evolution of member care, in keeping with changing needs and requests.
  • Document all visits, including deviations from approved care plans and prescribed intervention techniques.
  • Document member medical records and maintain electronic case management assessments for immediate and future use.
  • Identify development areas, including individual member enhancements and overarching LIFE Center opportunities for growth and expansion.
  • Counsel families in individual and group settings, and lead relevant activities to educate care participants.
  • Visit in-home members, assess needs, and develop/implement comprehensive speech/language care plans.
  • Perform general administrative duties, including completing and submitting required documents.

Qualifications: MSW from an accredited university, plus one year experience working with the elderly/frail population. Senior LIFE is an equal opportunity employer.
