Join Our Team as an Occupational Therapist at JAG Physical Therapy

We are seeking a highly skilled Occupational Therapist to join our team at JAG Physical Therapy. As an Occupational Therapist, you will play a crucial role in assessing patients' strengths and weaknesses to create individualized treatment plans that promote healing and independent living.

About the Role:
  • Collects and records patient care data that follows format and guidelines of the patient care service, regulatory agencies and third party payers which reflects an understanding of reimbursement systems and their impact on patient care.
  • Initial evaluation, daily notes, reassessments/progress reports, discharge plans and communication with members of the team.
  • Daily billing and follows department guidelines for productivity.

You will be responsible for preparing for evaluations, obtaining patient history, researching unfamiliar diagnoses, selecting and administering tests and measures, and synthesizing evaluation results to formulate diagnosis, goals, prognosis and plan of care.

Key Responsibilities:
  • Establishes treatment plan based on evaluation results, goals and best practice. Collaborates with interdisciplinary team, patients and patient's support system.
  • Assesses and re-assesses need for modifications to plan of care and goals.
  • Identifies need for consultations and/or refers the patient to another health care professional or health care services.
  • Initiates and completes discharge plan.
  • Provides education/training to patient and/or patient support system.

The ideal candidate will have excellent clinical decision-making skills, strong communication skills, and a commitment to delivering high-quality patient care. You will work collaboratively with our interdisciplinary team to ensure seamless delivery of care.
