POSITION SUMMARY:

As a caregiver at Homewell Care Services Brookfield, you will provide essential support to clients in their daily lives, following a personalized care plan developed by our Care Manager. Your role may encompass direct personal care as well as various companion and homemaking tasks, including light cleaning, meal preparation, grocery shopping, running errands, and offering companionship.

REQUIRED QUALIFICATIONS:

· A minimum of 1,000 hours of experience in long-term care with elderly or chronically ill individuals, equivalent to approximately 6 months of full-time or 12 months of part-time work. Exceptions may apply for newly certified CNAs or HHAs, or those providing only companionship and housekeeping services.

· Preferred certification as a CNA or HHA.

· Current TB test documentation is required.

· Must be physically capable of fulfilling the responsibilities of this role.

· Reliable transportation is necessary; if client transportation is involved, a valid driver's license and proof of automobile insurance are required.

· Strong communication skills are essential.

  • Exhibit maturity, compassion, punctuality, and dependability.

· Successful completion of a national criminal background check and any state-specific requirements.

· Provide verifiable professional and personal references.

KEY RESPONSIBILITIES:

· Comprehend and effectively implement the client's care plan.

· Assist clients in accordance with their care plan, which may include: companionship, meal preparation and service, light housekeeping, transportation, shopping, medication reminders, bathing, personal hygiene, dressing, feeding, mobility assistance, positioning, transferring, toileting, and protective supervision.

· Report any client concerns, changes in care, or emergencies to the Care Manager promptly.

· Communicate any safety issues to the Care Manager or Staffing Coordinator.

· Adhere to company infection control protocols; inform the office if Personal Protective Equipment (PPE) or supplies are needed.

· Follow ClearCare Telephony instructions for clocking in and out for all scheduled shifts and accurately update task statuses for each client before leaving.

· Comply with the company dress code as outlined in the Employee Handbook.

· Arrive at work punctually and prepared for the day.

· Engage in ongoing training as required by client care needs and changes in company policies.

· Attend staff meetings as scheduled.

· Abide by all company policies and procedures as detailed in the Employee Handbook.

SUPERVISION: The caregiver will be supervised by the Care Manager and will receive scheduling direction from the Staffing Coordinator.

PHYSICAL DEMANDS: The role involves repositioning and transferring clients, utilizing proper safety techniques and equipment, and traveling to clients' residences using reliable transportation.
