We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Home Health RN Case Manager. As a key member of our care team, you will be responsible for providing patient-centered care to individuals in their homes, promoting health and independence through teaching and rehabilitative measures.
Key Responsibilities:
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Makes referrals to other disciplines as indicated by the patient's identified needs or documents rationale for not doing so.
- Promotes patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques.
- Regularly communicate patient progress to the clinical manager and care team.
- Plans and provides staff education.
