Job Summary

We are seeking a highly skilled and compassionate Social Worker I to join our team at Houston Methodist. As a key member of our healthcare team, you will provide comprehensive clinical social work services to patients and their families, ensuring a seamless transition from hospital to home.

Key Responsibilities
  • Collaborate with physicians and the interprofessional healthcare team to identify and address psychosocial issues affecting patient care.
  • Develop and implement comprehensive psychosocial treatment plans, utilizing evidence-based practices and clinical social work diagnoses.
  • Provide crisis intervention, brief individual, marital, and family therapies, as well as patient, family, and caregiver groups.
  • Screen and identify patients and family members at risk for abuse, neglect, and domestic violence, as well as mental health and substance abuse issues.
  • Complete thorough assessments, identifying social determinants of health and developing plans to address these factors.
  • Establish mutual educational goals with patients and families, providing resources and planning for care after discharge.
  • Modify care based on continuous evaluation of the patient's condition, using problem-solving and critical thinking.
Requirements
  • Master's degree in Social Work (MSW) from an accredited program.
  • Licensed Medical Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in the State of Texas.
  • 0 years of experience, with new graduates and those with hospital internship experience preferred.
